Turning Point is a facility dedicated to addiction treatment located in Melbourne, Australia. The center employs a variety of forms in its operations, including initial screenings, case summaries, assessments, and reviews. Our brief was to enhance the interactivity and user-friendliness of these forms for staff and clients. Utilising Acrobat Pro, we developed fillable fields, buttons, and drop-down menus to streamline the forms.

Additionally, we used Acrobat JavaScript to enhance functionality and enable features such as full or partial resets of text fields, the ability to save and print the completed forms, calculate scores based on user input, and a one-click option for submitting results via email.
